There are many Realms and the Elder Devil Realm is one world in the Middle Realm.
The Elder Devil Realm is a realm that exists in a close spatial interface to the Spirit Realm. It is where those who cultivate demonic and devilish cultivation arts ascend to from their original mortal world. Situated above the Lower Realms, the Elder Devil realm has no Spiritual Energy. Instead it possesses dense Devilish Energy. Cultivators in the Lower Realms embarking on the Devil Dao ascend to this realm instead of ascending to the Spirit Realm. Also known as the Holy Realm.
The Elder Devil Realm isn't what we would imagine at least some parts of it, it is full of demonic beings with harsh environments but also has towering palaces and magnificent cities. The realm is overseen by powerful Grand Ascension powerhouses known as Sacred Ancestors.

As Wang Ning describe the elder realm in her own words, more or less its a place that is ten times more dangerous than the lower realms most vile continents, its very harsh and competitive due to some lack of cultivation resources. The Elder Devil Realm is where the Stemborer Queen was sealed by immortals from the True Immortal Realm in ancient times, as such it has negative impact on the realm where some locations within the devil realm is uninhabitable, this lead to the Higher ups having planned to conquer a part of the Spirit Realm.
